Actually, at our med school, they encouraged our class to write our own oath to be taken upon graduation. Talk about fluff! And many of my classmates don't even know that the Hippocratic Oath explicitly excludes abortion.
Many doctors don't take the Hippocratic Oath anymore simply because it would make them hypocrites, like you said. Instead they take some other oath, or a version of the Oath with the abortion part removed.
